For Patients
The Mālama Kidney Center program is specially designed to help you and your loved ones understand how the kidneys work, what your doctors are looking at and why, and what steps you can take to support your kidneys.
Our goal is to help you learn how to support your kidneys, be an active patient, and advocate for your own health. We are excited to begin this journey with you!
Care Management
Your dedicated care manager can help you in many ways. Our goal is to provide regularly scheduled, ongoing, and individualized patient education, care coordination, symptom monitoring, and medication review. Our team does not change or prescribe medications but helps you to understand what your medications are for and how they work. In addition, home visits may be arranged when necessary. Your care goals and plans are communicated with each of your health care providers to ensure the best care possible.
Patient Education
You and your loved ones receive individualized education sessions that provide information about current & chronic kidney disease (CKD) care, treatment options (dialysis, transplant, and conservative care), medication management, and advance care planning.
CKD Medical Nutrition Therapy
Your diet plays an important role in the care of your kidneys and can be one of the most difficult things to understand. Our Registered Dietitian can help you learn to make healthier food choices through
education and counseling about a CKD-friendly diet. You will learn skills such as proper meal planning and food label reading.
Behavioral Health
We encourage you to meet with our behavioral health professional (Psychologist or Licensed Clinical Social Worker) because we recognize that living with any chronic disease is stressful. We understand that your beliefs, feelings, and current behaviors play an important role in taking better care of your physical health. These professionals provide health coaching, counseling, and behavioral health therapy.
